Camp Release of Liability • Release of Liability I hereby waive, release, and discharge Neighborhood Comedy Theatre, Phoenix Improv LLC, and all associated instructors, staff, and volunteers from any and all claims, liabilities, demands, or actions relating to any injury, illness, or damages that may occur during or as a result of my child’s participation in the Camp.

Assumption of Risk • Assumption of Risk I understand that improvisational theatre activities involve physical movement, group interaction, vocal expression, and other activities that may carry risks of injury. I acknowledge that I have been informed of these risks and that my child is participating voluntarily. I confirm that my child does not have any medical or physical conditions that would increase their risk of injury during Camp activities.

Camp Medical Authorization • In the event of an emergency, I authorize NCT staff to obtain medical treatment for my child. I understand that I am fully responsible for any costs related to such treatment.

Camp Safety & Behavior Expectations • I agree that my child will follow all safety instructions given verbally or in writing by NCT staff. I understand that unsafe or disruptive behavior may result in my child being dismissed from the Camp without refund.
